These Terms of Service govern access to Frederick Recovery services, including pilot onboarding, configured recovery kits, and access to the Frederick Recovery platform. By placing an order, requesting a pilot, or using the service, the clinic agrees to these terms.

1. Service Overview

Frederick Recovery LLC provides a clinic-facing post-discharge recovery support system that may include configured recovery kits, digital patient education, patient check-ins, symptom monitoring, escalation workflows, and clinic dashboard access. Service details, rollout timing, and pilot scope are confirmed through direct clinic communication.

2. Pilot Pricing

Pilot pricing is $15 per standard box and $10 per add-on pack, with the full recovery platform included free during the pilot. Those base rates do not increase while the account remains active. The only adjustments allowed are documented vendor cost increases passed through at cost with no markup. If an account lapses for more than 90 days, it re-enters at current rates.

Quote requests and pilot conversations help confirm rollout scope, procedure fit, add-on pack selection, and operational details before pricing is finalized in writing.

3. Orders, Deposits, and Customization

A $50 reservation deposit is refundable until branding proofs are approved or production begins. Once custom branding is approved, items are non-refundable. Refund requests must be submitted in writing. Shipping costs are non-refundable.

Configured materials, insert packs, and clinic-specific customization may require additional approvals before production or launch.

4. Clinic Responsibilities

The purchasing clinic is solely responsible for all medical decisions, patient instructions, treatment protocols, clinical escalation standards, and determination of whether Frederick Recovery is appropriate for its patients and workflow. The clinic is also responsible for obtaining any patient consents or notices it requires.

5. Medical and Product Disclaimer

Frederick Recovery LLC does not provide medical advice, diagnosis, or treatment. Recovery kits are intended as supplemental aftercare supplies only. Patients may experience sensitivities or reactions to materials, and clinics are responsible for evaluating patient suitability and providing appropriate guidance.

6. Platform Access and Acceptable Use

Clinic users must use the platform only for authorized clinic purposes and may not misuse the service, interfere with security controls, attempt unauthorized access, or use Frederick Recovery in a way that violates applicable law, patient privacy obligations, or clinic policy.

7. Privacy and Compliance

Use of the service is also subject to the Frederick Recovery Privacy Policy. Frederick Recovery describes its workflow as HIPAA-aligned and does not claim certification or accreditation unless expressly documented. 8. Availability and Changes

Frederick Recovery may update, refine, or improve the service over time. Pilot timing, feature availability, and fulfillment timing may depend on rollout planning, clinic approvals, production timing, or vendor availability.

9. Limitation of Role

Frederick Recovery is intended to support structured post-discharge visibility under clinic control. It is not a substitute for clinical judgment, emergency response, or direct patient care. Clinics remain responsible for review and action on any information surfaced through the service.
